<br />M-95-026/Adequacy <br />4/24/95 <br />page 2 <br />EXHIBIT B - INDEX MAP <br />The two index maps are well prepared except that the specific areas <br />outlined on each are different than what the permit area is. The <br />outlines presumably depict the 120-acre county-owned parcel within <br />which the permit area is located, but the outline should depict <br />only the permit boundary. Copies of the same maps may be submitted <br />to replace these originals, but please show a corrected outline. <br />EXHIBIT C - PRE-MINING AND MINING PLAN MAPS <br />There are two maps submitted at this point within the application <br />materials. One is labeled "Mining Plan Map" and the other (the <br />large one) is untitled, but presumably is the pre-mining map. That <br />map should be specifically labeled. <br />The map we consider the pre-mining map (the above-mentioned plat <br />from Shy Surveyors) contains some, but not all, information about <br />the adjacent landowners' names. Ownership must be shown for all <br />properties within 200 feet of the permit boundary. The names for <br />landowners adjacent to the east side, north side (across the county <br />road) and south side of the permit boundary should be shown. The <br />permit boundary itself should be labeled as such. No mining- <br />related features need to be shown, unless they are pre-existing on <br />the site. I am assuming that within the area labeled "topsoil <br />storage area" there is no topsoil actually there yet. Please <br />ensure that all significant structures (including underground <br />utilities) and their ownership are shown. <br />The mining plan map (as labeled) is of too small a scale to <br />adequately depict sufficient detail of the activities in the four <br />phases. Its scale should, in fact, be somewhat larger than that of <br />the plat discussed in the preceding paragraph, in order to allow <br />space for the required details. The separate types of information <br />which should be shown on this map include: acreage of each phase <br />(the amount in Phase I differs between the two maps), direction of <br />mining in each phase, permit boundary (labeled), drainage pattern, <br />sedimentation and runoff control structures, and areas where <br />stockpiles will be located. Be reminded that all mining <br />activities, including the stockpiling of material, roads, storage <br />and fueling areas, structures, and reclamation must be within the <br />permit boundary. It is difficult to determine between the two maps <br />in this exhibit whether the road along the east side is in or out <br />of the permit area. The location shown for topsoil is about 0.48 <br />acre in size, but is outside the presumed permit boundary. Either <br />the stockpile location or the presumed boundary line must be <br />relocated. <br />EXHIBIT D - MINING PLAN <br />Please be more precise concerning the acreage figures mentioned in <br />the narrative. what types of erosion prevention (ie., berms, <br />
