the # 7 Nonh Escape Shaft.
Date. Location and Tvpe of Administrative Proceedings: A proposed assessment was sent to TCC on May 22,
1996 and proposed civil penalty- of $1,650.00. TCC requested an assessment conference on June 17, 1996.
The assessment conference was held on July 24, 1996 and a civil penalty of $1,150.00 was proposed based on
the information presented during the conference. The Division sent TCC a Settlement Agreement on July 26,
• 1996; TCC signed and retumed the agreement on August 5, 1996.
Current Status: NOV terminated on June 12, 1996
Actions Taken to Abate Violation: The drainage control for road, pad, and outslopes was restored, silt fences
were repaired, gullies were repaired, and sediment below the silt fence was cleared out.
