Please respond and note that any changes in the mining and reclamation plans <br />brought about by the SUR process may need to be incorporated into the DMG Regulaz 112 Application. <br />CDOT Highway Access Permit -The City of Black Hawk has indicated that the applicant is not in <br />compliance with applicable local permits, licenses, and approvals as the applicant does not have a <br />Colorado Department of Transportation (CDOT) state highway access permit in order to access the subject <br />property and conduct their mining operation. Please see the attached City of Black Hawk objection letter <br />received by the Division on December 30, 2004. Please respond. <br />6.4.19 EXHIBIT S -Permanent Man-made Structures <br />3 Structures -The applicant has indicated on page 34, Exhibit S, that there are no permauent man-made <br />structures within 200 feet of the proposed mining azea, but that there aze three (3) structures within 200 <br />feet of the main access road.
