May affect natural resources of the U.S. <br />2004 SPCC <br />The decision to report a spill will be made by the Environmental Manager or his designee. <br />Verbal notifications are required according to the procedures described below. Notification to <br />the agencies for various spill conditions, shown in Table 5, is required only if an emergency <br />condition exists which the Environmental Manager believes requires assistance from outside <br />agencies. Agencies may be contacted otherwise if it is determined that a courtesy call to the <br />agency is appropriate. <br />If written follow-up to the spill is requested by an agency following the verbal notification, <br />provide them with a copy of the Spill Information Form provided in Appendix 5. It is expected <br />that such a request will only be made by the Colorado Department of Health and Environment <br />(CDPHE). Refer to the Trapper CDPS permit for details. The Environmental Manager will <br />review the completed form used for verbal reporting, revise it if necessary, and submit the form. <br />• 7.4 REPORTING LARGE AND FREQUENT SPILLS <br />If a spill of more than 1000 gallons of oil products occurs in any event, or if there are two spill <br />events in a twelve month period, each with greater than 42 gallons spilled, then § 112.4 requires <br />submitting to the Regional Administrator of EPA the following information: <br />I. Name of facility; <br />2. Your name; <br />3. Location of facility; <br />4. Maximum oil storage or handling capacity of facility; <br />5. Corzective action and countermeasures you have taken, including a description of equipment <br />repairs and replacements; <br />6. A description of the facility, including maps, flow diagrams, and topographical maps; <br />7. The cause of such dischazges, including a failure analysis of the system in which the failure <br />occurzed; <br />8. Additional preventive measures taken or contemplated to minimize the possibility of <br />recurzence, and <br />9. Such other information as the Regional Administrator may reasonably require pertinent to <br />the plan or discharge. <br />I~ <br />23 <br />
