See 40 CFR 434.11(j) for the complete <br />definition. <br />C. REPORTING REQUIREMENTS <br />1. Reoorting - Process Water <br />Monitoring results obtained during the previous 3 months shall be <br />.summarized and reported on Division approved discharge monitoring report <br />forme, postmarked no later than the 28th day of the month following the <br />completed calendar quarter. For example, for the 1st quarter (January, <br />February, and March) the quarterly report must be received at this <br />office postmarked no later than April 28. If no discharge occurs during <br />the reporting period, "NO Discharge" shall be reported. <br />The first original copy of each discharge monitoring report (DMR) shall <br />be submitted to the Division at the following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each report form shall be submitted <br />to the following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch BWM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />2. Reoortino - Stormwater Annual Reoort <br />The permittee will be required to submit an annual report, covering <br />January 1 through December 31 of each year, on the overall compliance <br />with the SWMP. The annual report will contain, at a minimum: <br />a) Name of permittee, address, phone number, and permit certification <br />number. <br />b) A report on the facility's overall compliance with the SWMP. <br />Amended lU/13/94 Effective 11/13/94 <br />
