• • PART III <br />Page 16 of 19 <br />PERMIT No: co-0035653 <br />A. SPECIAL REQUIREMENTS <br />1. No[ withstanding the provisions of PART II, paragraph A-1 the permittee <br />- shall submit a written request to the Colorado Water Quality Control <br />Division and the U.S. Environmental Protection Agency for designation of <br />discharge points. The designation of present discharge points': shall be <br />submitted to the Permits Section, Water Quality Control Division in not <br />more than 90 days following the effective date of this permit. New <br />discharge point designations shall be submitted in not less than 90 days <br />prior to the proposed time of discharge. The request shall be accompanied <br />by area maps which shall meet the specifications set forth below. Each <br />discharge point shall be numbered consecutively (001, 002, 003. etc.) and <br />designated on the map. The Division shall approve or deny proposed <br />discharge points in writing, or may in its discretion require that the <br />permittee accept different effluent limitations as a condition of approval <br />or submit a new permit application. Approval of additional discharge <br />points shall not constitute a permit modification, and no public notice <br />or hearing will be required, unless the WQCD, in its sole discretion <br />determines that the chance is significant. <br />2. Area maps shall be made from USGS topographical maps (7.5 or 15-minutes <br />series) or other appropriate sources as approved by the Colorado Water <br />Quality Control Division. Each revised area map shall be 8-I/2 inches <br />by 11 inches and reproducible by rapid printing methods or as approved <br />by the Division. Where additional 8-1/2 x 11 maps are required to show <br />the area of operation, they shall be numbered and a key shall be shown <br />on the first map. The first map section shall have the company name, <br />mine/job name, address, and NPDES number clearly printed thereon. Also, <br />one line of latitude and one line of longitude shall be marked on each <br />map section. The area map(s) shall delineate the following, using the <br />graphics as indicated: <br />(A) Existing area of operation (Solid Outline) <br />(B) Existing point source . (Solid Triangle) <br />(C) The projected area of operation for the life <br />of the NPDES permit --------------------------- (Dashed Outline) <br />(D) The projected point source for <br />the life of the NPDES permit ~ (Opened Triangle) <br />3. The permittee shall submit a written request [o the Colorado Water Quality <br />Control Division and the U.S. Environmental ,Protection Agency to deactivate <br />~ a discharge point. The Division will grant or deny the request in writing. <br />The Division's decision will be based on whether grad'+ng of the disturbed <br />area has been completed to return the earth to the desired contour and <br />reclamation work has begun. If the request to deactivate is granted, the <br />point in question shall not be subject to any NPDES permit requirements. <br />`(except 001, 002, 003, 004, and 005) <br />2/81 <br />
