Stormwater Manaeement Plan (continued) <br />d) The plan shall include a list of personnel directly responsible <br />for stormwater management. The plan shall describe the procedures <br />[o be followed for personnel to judge compliance with the SWMP. <br />e) The plan shall provide for proper trainine of affected employees <br />in techniques and procedures that will assure achievement of all <br />seated goals. <br />f) The permittee shall amend this plan at any time if there i.s a <br />change in operation that will or may result in a deterioration in <br />the quality of pollutants addressed by [his plan, or if the plan <br />is shown to be ineffective in achieving the specified goals. A <br />copy of the amended plan shall be submitted co the Division and <br />[he CMLRD within 30 days, along with an explanation of the reason <br />for the amendment. If the Division finds the amended plan to be <br />inadequate, or if the procedures established are shown to be <br />ineffective once in place, the permittee shall develop and employ <br />new procedures. <br />g) The permittee shall submit ar.nuai reports due on the anniversary <br />date of the speciE'_c facility certification. Each report shall be <br />signed and certified (sze Part I.C.6.j and shall include a <br />statement affirming that a thorough inspection of all affected <br />operations covered under this plan has occurred at Least once per <br />quarter during the period covered by the report. ?f the <br />inspection detects complete conformance with the plan, this shall <br />be so stated. If the inspection shows the plan [o be <br />inappropriate or ineffective in any area, the annual report shall <br />address how the permittee has corrected or intends to correct this <br />deficiency. A11 annual reports shall become a pare of this permit <br />by reference. <br />h) whenever a facility or a portion of a facility enters the <br />Bost-minims phase, it is likely that the overall configuration of <br />the site will change considerably. As the land is reclaimed, <br />roads, railroad lines and structures will be removed, and <br />significanr. reshaping of the surface will occur. Prior co [he <br />time when new vegetation becomes established, [he surface areas <br />will be subject to potentially significant erosion. The permittee <br />will need to rake adequate measures in order to minimize this <br />erosion. Therefore, at the time of post-mining, the permittee <br />shall amend the plan to include specific procedures to. minimize <br />erosion and other factors chat contribute to the discharge of <br />total suspended solids and other pollutants from newly reclaimed <br />areas. A copy of the amended plan shall be submitted to the <br />Division and the CMLRD with the amendment request co change from <br />active to post-mining requirements. <br />
