SHELTON, Director <br />February 4, 1986 <br />Mr. David Bacon <br />Mr. Glenn Southwick <br />7816 West County Road 24-H <br />Loveland, Colorado 80537 <br />RE: Permit Application - Ma sonville Stone - File No. M-86-007 <br />Dear Sirs: <br />We have reviewed the above-referenced application and have the following <br />questions and recommnendations. These are intended to bring the <br />application and the mining operation into full compliance with the <br />Colorado Mined Land Reclamation Act, 34-32-101 et seq. Please provide <br />your response as soon as possible, but no later than February 19, 1986. <br />The application will be considered at the February 26-27, 1986 Board <br />meeting if your response to this letter is adequate, if you have proof of <br />local government compliance and if proof of landowner notification, <br />adjacent landowner notification and legal proof of proper newspaper <br />publication are submitted prior to the February 1986 Board meeting. <br />Additionally, the "Small Operator's Permit Worksheet" which was submitted <br />on January 15, 1986 is helpful, but this worksheet is not intended for <br />use as your actual application. Instead, the actual appliation should <br />incorporate the information on the worksheet and any other relevant <br />information into descriptions with separate sections devoted to each <br />required exhibit. Please provide a revised application which <br />incorporates 1) the worksheet information, 2) the other submitted text <br />and 3) your responses to this adequacy letter. The purpose of this <br />revision is to provide a single concise reference for both the operator <br />and the staff, such that all procedures and commitments are clearly <br />outlined. <br />Exhibit A - Legal Description <br />1) Provide a separate Exhibit A page which describes the site legal <br />description. <br />Exhibit B - <br />-' 1) All aspects of the mine including the actual quarry, stockpile and <br />equipment areas and topsoil source areas must be included in the <br />total permit area. Are all of these aspects of your mine within <br />two (2) acres?
