<br />Please clarify why Exhibit M indicates that a permit for above ground storage tanks <br />is "to be assigned" but the stormwater application indicates that fuels will not be <br />stored on site. <br />21. Exhibit S -Permanent Man-Made Structures of the application provides statements <br />indicating that no impact to the listed structures are anticipated. However, the <br />statements contained in Exhibit S of the application do not provide an appropriate <br />engineering evaluation that demonstrates that such structure shall not be damaged <br />by activities occurring at the mining operation. Please submit the information <br />required by Rule 6.4.191a) or (b) for all significant, valuable or permanent man- <br />made structures located within 200' of the permit boundary. <br />The applicant has submitted copy of a damage compensation agreement for <br />property fencing, the Home Ditch, the gravity drain pipe and access road, signed by <br />the applicant and landowner. <br />If DMG determines that the applicant's response to the above adequacy review items 1, <br />2, 3 and 4, constitutes an amendment to the application, the decision date will be <br />extended by a minimum of 45 days, as required by Rule 1.8.411-. If DMG determines <br />that the applicant's response to the above adequacy review items constitutes a technical <br />revision to the application, the decision date will be extended by a minimum of 20 days. <br />Please review Rules 1.1(61, 1.1149) and 1.8.
