Lis[zd Eovironmerital Consultam Program <br />• <br />~ttp://oiLcdlestate.co. us/cost'7a20guidelines/con rg. html <br />~ Comply with Colorado's Rensonab/e Cost Guidelines, including providing to any client who <br />may seek reimbursement from the Petroleum Storage Tank Fund a copy of the Guidelines <br />together with a signed "Client Notification" form MHN (at § 2-4 of the Guidelines) showing <br />that the client has been advised that any otherwise allowable costs billed at rates exceeding <br />those set forth in the Guidelines may not be reimbursed at a rate higher than the Guidelines <br />rate unless considered justified by the Petroleum Storage Tank Committee, submitting <br />invoices that are compatible with the format established in the Guidelines, and not mazking up <br />materials or services provided by any company or person with whom the listed company has a <br />financial interest. <br />~ Ensure that all activities billed at labor rates for a principal, senior engineer/scientist, project <br />manager or project engineer/manager will be conducted by a listed environmental scientist. <br />2-1-6. ENVIRONMENTAL SCIENTIST <br />To become a listed environmental scientist for Fund reimbursement purposes, a person must complete the <br />individual form (at § 2-3 of the Guide[irtes). To be eligible, a person must meet the following minimum <br />qualifications: <br />(a) Have at least five (5) yeazs of qualifying experience and meet the requirements of either <br />(b)(I) or (b)(2): <br />(b)(1) Have a recognized professional license or certification such as professional engineer, <br />professional geologist, professional geological scientist; <br />-or- <br />(b)(2) Be a graduate of an institution of higher education that is accredited by a regional or <br />national accrediting agency, and have successfully completed at least 30 semester (45 quarter) <br />hours of undergraduate work in one of the following: engineering; biological, chemical, <br />environmental, or physical science; or industrial hygiene. <br />If a listed environmental scientist leaves to form his/her own company, that company must become listed <br />with the OIS. <br />2-1-7. REVOCATION OF COMPANY LISTED STATUS <br />A company's listing status may be revoked by the Petroleum Storage Tank Committee for any of the <br />following reasons: <br />(a) It no longer has a listed environmental scientist on staff. <br />(b) It is convicted, enters into a plea agreement, or enters a plea of nolo contendere to any <br />crime involving a violation of Colorado or federal environmental laws and regulations, <br />including any violation of laws or regulations governing Colorado's Petroleum Storage Tank <br />Fund. <br />(c) It provides false information on any documents submitted [o support a Fund <br />reimbursement application, such as technical reports, invoices, and affidavits of payment, if <br />the company had reason to know the information was false. <br />(d) It repeatedly violates any other provision on the listing application form to which the <br />company agreed by signing the form. <br />2 0(3 6/30/99 ?:56 PM <br />
