INTRODUCTION <br />Choosing an environmental consultant is a serious responsibility. No other single factor within the Fund <br />Applicant's control will so significantly affect the cost, the effectiveness, and the duration of the <br />remediation project. <br />To assist persons in the consultant selection process, and to increase assurance that reimbursement from <br />Colorado's Petroleum Storage Tank Fund is for activities that are both technologically and cost effective, <br />the Oil Inspection Section (OIS) has developed a program to list environmental consulting companies and <br />environmental scientists. <br />2-1-2. DISTRIBUTION OF CONSULTANT LIST <br />The OIS will send the list of participating environmental consulting companies to any person upon request, <br />as well as to any person who reports petroleum contamination. <br />2-1-3. PROGRAM OVERVIEW <br />This is a voluntary program. No fee, testing, continuing education, proof of insurance, or renewal is <br />required. The requirements of the program apply only to activities for which reimbursement is sought from <br />Colorado's Pevoleum Storage Tank Fund. <br />Listing is by company. To be eligible to participate, a company must employ at least one person who <br />qualifies as a Listed Environmental Scientist (see § 2-1-6 of the Guidelines). <br />2-1-4. NUMBERING <br />The OIS will issue a listing number to the company and separate listing numbers to each environmental <br />scientist. <br />2-1-5. ENVIRONMENTAL CONSULTING COMPANY <br />To become a listed environmental consulting company, a company must complete the company form (at § <br />2-2 of the Guide(i/res), sign it before a notary public, and return to the OIS with all appropriate <br />attachments: <br />Properly completed individual environmental scientist form(s) <br />Company's quality assurance/quality control plan <br />Company's health and safety plan <br />In signing the form, the company certifies its agreement to perform the following to the best of its ability: <br />Abide by all federal, Colorado, and local statutes, ordinances, regulations, guidelines, <br />standazds, practices, policies and other requirements pertaining to assessing and remedia[ing <br />petroleum contamination in soil and water. <br />Notify the OIS within 30 days of any change in information provided on the form. <br />I of? 6/30/99 2:56 PM <br />
