diversion dike may satisfy part or all of the storm water permit/storm water management
plan you may have to obtain from the Water Quality Control Division.
Exhibit E -Mine Plan/Reclamation Plan Mao
~~Y. Your Exhibit E map does not clearly describe the features associated with the mining plan
and the components of the reclamation plan. Please provide a replacement map that
shows these featur'eAS clearly and accurately. l /'
gcalC r i 1 >0 5 ~^C~r !j~ Ad oar cus~~G//r'c 9- Ga`.~yi^~%ry-~-rr~~r _ f
Your map must be drawn to scal no smaller than 1 nch equals 50 feet. Please provide a
scale on the map and draw the map to that scale. /~i~l, f<at'a rs ~ ~ ~= 5"O ~
~f{'f-3. Is the 9.025-acre permit boundary the same as the prop_e/rt~y line? If so, please label as
such.
0~4. The map is not clear as to what areas will be affected by mining activities. Is the 2.75-

acre clay extraction area the only land to be affected within the permit area or will
additional land be affectedT Arrows on the mine map seem to indicate that the entire
permit area will be affected. Accurately delineate each area of affected land.
tl~5. On the map, label the names of owners of record of the surface of the affected area and
o'f~,fthe land within 200 feet of the affected area/s.
~~- 6. by utline and label on the map the pit boundary, topsoil stockpiles, overburden stockpiles,
O ~`/
any, and any necessary diversions (must be included in the permit area).
(~('~7. Arrows on the mine plan/reclamation plan map are confusing. They indicate that the
entire permit area will be mined. However, the mine plan states that only a small area
(2.75 acres) will be mined for clay. Please clarify.
Q~`$. You must outline and label existing disturbance within and/or adjacent to the permit
boundary. You have outlined an area in the northwest corner of the map. It is labeled but
it is not legible. Please provide the label on your replacement map. In the northeast
corner of your map, an area is labeled as "active mining." Is this area being mined at the
present time? If so, please explain the operation. Who is mining? What dates did mining
take place? What was(s the mined material used for? Is this the area proposed for clay
extraction)
0~9. Indicate the gradient of all reclaimed slopes (horizontal:verticaq sufficient to describe the
post-mine topography.
G~t10. Will any overburden material be replaced back into the pit at reclamation) If so, how
much?
Exhibit F -Permits and Licenses Required
~~/LY. You have stated that the only permit required is the "Mining and Reclamation permit with
the Colorado Mined Land Reclamation Board." You must also apply for a storm water
permit from the Colorado Health Department Water Quality Control Division.
