If the site is in temporary cessation orpost-mining, please state. The Division deft <br />these terns as follows: <br />Exploratory - An exploratory mine is one m which analysis is being done to detetmhme if active mining is economical. Any active mining <br />occurnd prior M 1972 or the site was previously reclaimed and released from bond by the Colorado Division of Minerals and Geology (CDM <br />Active - An active mine is one in which coal is being mined or has been mined s'mce 1972. A facility is considered active by the Division aril <br />CDMG releases the facility from bond. <br />Temporary Cessation -Temporary cessation is a CDMG teen which addresses those mores which are not actively ptnducing coal but whirl <br />have not commenced final reclamation or closure of the site. The Division considers such factlhies as active. <br />Post-Mining -Amine or an area of a mine U wnsidered to be m post-mining when the surface area of a coal mine has been returned to the <br />required wntours end which vegetation (reseeding) work has commenced. An urea is considered to be in post-mining until the bond has beet <br />released from CDMG. <br />If a mine is explormory, it is requested that information on the quality of the stream and any discharges be submitted. Failure to do so may result in <br />difficulties later should you request inactivation. <br />Item 8,9 - Self explanatory. <br />Item 10 - A discharge is intermittent unless U occuts without interruption during the operating hours of the facility, except for maintenance, process change c <br />sim>7ar shutdown. A discharge is seasonal if it occurs Daly during certain parts of the year. <br />Item 11 - This map is intended [o serve as an area map for inclusion in ilea pemth. A legible 82 x 11 inch submittal is required. A larger map maybe incl <br />for reference if [he permittee wishes. If maps me not supplied, issuance of vour permit will be delaved. <br />Item 12 - This is a facility sketch for inclusion m the permit. A legible 82:11 inch submittal is required. If sketch is not supplied, or if it is larger than 8 <br />11 inches, issuance of vour permit will be delaved. <br />Uem 13 - The tine drawing should show generally flee route taken by water in your facrlity from intake to discharge. Show all operations contributing <br />wastewater, including process and production areas. The water balance should show average flows and aU significavt losses of water to products, t <br />atmosphere, dust control and discharge. Use actual measurement whenever possible, otherwise use your best estimate. If a water balance is not <br />appropriate, please indicate. <br />Item 14 • Mark which of the other permits/progtams the facility has obtained or is is the process of obtaining or is subject to regulation under. <br />See items 37 and 38 regarding Smrmwater requirements. <br />Item 15 - Self explanatory. <br />Item 16 - List here those chemicals or materials added ro the water prior to, during, and after the water is used and prior to discharge. Also list chemicals or <br />materials used in the treatment of the wastewaters. <br />Item 17 - If appiicabh;, list tbose toxic products that are used in the manufacturing process or are produced by the manufacturvtg process. Do not include the <br />substances listed in darn 16. <br />Item 18 - Self explanatory. <br />Item 19 - List any improvements m the wastewater system which are required or which are being undertaken to ensure future compliance with environment <br />regulations. The Division may request additional information concerning these changes during the application review process. <br />Item 20 - If land application, defined as any discharge being applied [o the land for treatment purposes, is practiced or proposed the Division needs <br />appropriate information to undetstmtd the operation and make a judgement as m possble impact on surface waters, If not identified elsewhere, <br />identify the nearest surface watets or dry stream bed. Land application of waters U subject to the Colorado Division of Minerals and Geology <br />review and approval. Please include the information submitted to CDMG to obtain such approval and a copy of the letter received granting such <br />approval. <br />Item 21 - List the ou[fa0 number for each discharge point. List all sources of wastewater for each outfaU and give the 30 day average flow and deity <br />maximum flow. You may estimate the flow contributed by each source if no data is avmlable, and for stormwater, you may use any reasonable <br />measure of duration, volume or frequency. Describe each treatrnen[ unit. Indicate the 10-year, 24fiour equivalent volume used in designing the <br />treatment system and the system's actual volume, excluding solids retention and any "permanent pool" that maybe provided. Indicate ff extra <br />capacity is provided for mine water and/or other nonstorm related flows and bow this volume was determined. Indicate what type(s) of <br />discharge structure each outfaU has and Uow flow is discharged - whether U discharges automatically or manually. If your flows vary <br />significantly or if you anticipate significant changes ht flows during [he next 5 years, specify which flows will change and explain why they will <br />change. Describe the ultimate disposal of any solid or Uquid waste not discharged. Use additional pages as necessary. <br />
