Freauencv and Samole Tvne (continued) <br />(t) Oil and Grease Manitorine <br />For every ou[fall with oil and grease monitoring, in the evrnt an oil sheen or floating oil is observed, a grab sample <br />shall be collected, analyred, and repotted on the appropriate DMR. [a addition, cortective action shall be takes <br />immediately to mitigate the discharge of oil and grease. A description of the corrective anion takes should be <br />included with the DMR. <br />2. Reoortine of Data <br />Reporting of the data gathered in compliance with Part l.B.1 shall be as a ~ rterly basis. Monitoring results shall be <br />summarized for each calendar quarter sad repotted on Division approved discharge monitoring report (DMR) forms <br />(EPA form 3320-1). The forms shall be mailed to the agrncia listed below so thry ace received no lazer than the 28th <br />day a[ the month following the end of the quarter (for example, the DMR far the first wleadar gttarter must be received <br />by the Division by April 28th). If no discharge omtrs during the reporting period, 'No Discharge' shall be reported. <br />The DMR forms consist of four gaga -the top 'original" wpy, and thra attached no-carbon-required wpia. After the <br />DMR form has been filled ou[ and signed, the four copies must be separated sad distributed as follows: <br />T1te first on final signed copy of each ducharge monitoring report (DMR) shall be submitted to the Division az the <br />following address: <br />• Colorado Department of Public Health and Environment <br />WQCD•PE-B2 <br />4300 Cherry Creek Drive South <br />Deaver, CO 80222-1530 <br />The first duoliwte signed copy of each discharge monitoring report (DMR) shall be submitted to the following agrnry: <br />U.S. Enviroamenral Protettion Agrnry <br />Terhniwl Enfotument Program SENF-T <br />Office of Eoforcemrnt, Compliance Assistance and Eaviroameatal Justice <br />999 18th Street, Suite 500 <br />Deaver, CO 80202-2466 <br />The third and founh wpia are for the permitter records. The Discharge Monitoring Report forms shall be filled our <br />accurazely and completely in accordance with requirements of this permit sad the iastructians on the forms. They shall <br />be signed by as authorized person as idrntified in Part I.D. <br />3. Acute WEI' Testing -Outtalk 002. 004. Oll and Ol3 <br />(a) Testin¢ and Reoortin¢ Reouiremrnts <br />Tats shall be done az the frequrnry listed in Part I.B.1. Tat results shall be reported along with the Discharge <br />Moni[oring Report (DMR) submitted for the reporting period doting which the sample was takrn. (i.e., WET rating <br />results for the first calendar quarter ending March 31 shall be reported widt the DMR due Apri128J The results <br />shall be submitted on the Acute Taxiciry Tat report form, available from the Division.
