Long <br /> Division Direuor <br />1 1 2c Application Preliminary Adequacy Review <br />The Division has completed the preliminary adequacy review for the Villa Grove Pit 11 2c permit <br />application. The Division cannot approve the application until the comments listed below have <br />been adequately addressed. Please respond as soon as possible, or at least by December 15, <br />2000, so the decision due date set for December 26, 2000 can be met. If you cannot adequately <br />respond by December 15, 2000, please submit a written request for an extension of the decision <br />due date. Please be aware, if your adequacy review response, or a written request for extension <br />of the decision due date is not received, by the Division, by December 15, 2000, this application <br />may be denied. <br />ADEQUACY REVIEW COMMENTS <br />1 . Please provide a copy of the CDOT access agreement to document the damage agreement for <br />the CDOT fence, culverts and Highway 285. <br />2. Please re-submit the damage waiver agreements with Quest communications and Saguache <br />County ensuring these agreements are also signed by J and K Limestone. This signature must <br />also be notarized. <br />3. Please demonstrate that the buffer zones will be adequately sized to hold the amount of <br />topsoil to be stockpiled there. <br />4. Per our conversation on November 21, 2000 it is the Division's understanding that all outer pit <br />slopes will be mined at final grade. Please revise the mining and reclamation plans to specify <br />this commitment. <br />5. It is assumed that the interior pit slopes will be vertical mining faces with a maximum length of <br />1150 feet. Costs for cut and fill reduction of these faces, to a 3:1 slope, will be included in <br />the Division's reclamation cost estimate. If these assumptions are incorrect, please specify <br />the maximum length and grade of interior highwall that will exist at any time during the <br />operation. <br />6. Please provide details of the scale construction, if it is not a mobile scale. Specifically, its <br />dimensions and whether it will have any permanent footers and/or stem walls. <br />
