By law, the Division is required to investigate any activities that may require a reclamation permit <br />under the Colorado Construction Materials Rules and Regulations. In order for the Division to ascertain whether a <br />reclamation permit is required or the activities are exempt from such regulations Division staff request you fill out this <br />questionnaire. <br />The Board has directed the Division to make a preliminary determination, regarding the activity and to advise you of <br />that determination. If the Division makes an initial determination that Dirt Merohant Construction needs a reclamation <br />permit, the Division will schedule a hearing before the Board for a final determination. <br />Please answer the following questions in as much detail as possible, It is recommended that responses be made on <br />separate paper to allow for as much information as possible. The more information staff has the better the preliminary <br />determination can be made. These are some of the questions the Boazd typically asks during such determination <br />hearings and aides the Division in its preliminary determination as well. <br />1. Please give the legal location of the proposed project and submit an index map that clearly delineates <br />the location of the proposed extraction site and the location of the nearest city, town, and county <br />location name. <br />2. Is the site of material extraction public or privately owned property? <br />3. What types} of material is proposed or has been extracted? <br />4. Has the extracted material been hauled offsite or has it been used on the same parcel of property <br />where the material is extracted? <br />5. What on site use will be made of the extracted material? <br />6. If the material has been hauled offsite, where has it been hauled and what is the intended use? <br />7.
