The Division of Minerals and Geology (Division) is required to make an approval or denial decision no later than <br />August 21, 2006, therefore, your response to the following adequacy review concerns should be submitted to the Division <br />as soon as possible. <br />Rule 1.6.2(11(81 -General Applicant Procedures <br />1. In accordance with this rule, please submit proof of publication circulated in the local newspaper and proof of the <br />public notices submitted to all owners of record of surface lands, easement holders, and owners of record of surface <br />lands that are within 200 feet of the boundary of affected ]and. Proof of Publication may consist of either a copy of <br />the last newspaper publication, to include the date published, or a notarized statement from the paper. Proo£ of Notice <br />may be by submitting return receipts of a Certified Mailing or by proof of personal service. <br />Rule 6.2.1(21(b1-Maps and Exhibits <br />2. Irl accordance with this rule, maps, with the exception of the index map, must be prepazed signed by a registered land <br />surveyor, professional engineer, or other qualified person. Please resubmit signed copies of the Pre-Mining Plan Map, <br />Mining Plan Map, and Reclamation Plan Map. <br />Rule 6.4.3 Exhibit C-1 -Pre-Mining Plan Map <br />3. In accordance with Rule 6.4.3(a), please identify the adjoining surface owners of record along the northwest permit <br />boundary. <br />4.
