The <br />Division of Minerals and Geology (Division) is required to make an approval or denial decision <br />no later than September 2, 1998, therefore, your response to the following adequacy review <br />concerns should be submitted to the Division as soon as possible. If you feel more time is <br />needed to complete your reply, an extension to the decision date can be granted by the Division. <br />This will be done upon receipt of a written waiver of your right to a decision by September 2, <br />1998 and request for additional time. This must be received no later than the deadline date. <br />1~ As required by Rule 1.6.5(2), please submit the proof of publication of the public notice <br />v circulated in the local newspaper. Proof of notice may be return receipts of a Certified <br />Mailing or by proof of personal service. <br />Please be advised that you may need a Stormwater Permit from the Colorado Department <br />of Public Health and the Environment, Water Quality Control Division. If any storm <br />water control structures aze required to be constructed at the mine site as a result of the <br />storm water permitting process, the operation and reclamation aspects of such structures <br />must be included in the Reclamation Permit. <br />3 Rule 6.2.1(2)(6) requires that maps and exhibits be prepared and signed by a registered <br />land surveyor, professional engineer, or other qualified person. Please have the maps and <br />exhibits signed in accordance with the Rule. <br />4. ~e application states that there are no significant man-made features within 200 feet of <br />e affected area. Please indicate on the Exhibit and permit text all structures, including <br />man-made features located within 200 feet of the affected area. <br />
