Reportin¢ of Data <br />Reporting of the data gathered in compliance with Part I.B.l shall be on a quarterly <br />basis. Monitoring results shall be summarized for each calendar quarter and <br />reported on Division approved discharge monitoring report (DMR) forms (EPA form <br />3320-1). The forms shall be mailed to the agencies listed below so they are <br />received no later than the 28th day of the month following the end of the quarter <br />(for example, the DMR for the first calendar quarter must be received by the <br />Division by April 28th). If no discharge occurs during the reporting period, "No <br />Discharge" shall be reported. <br />The DMR forms consist of four pages - the top "original" copy, and three attached <br />no-carbon-required copies. After the DMR form has been filled out and signed, the <br />four copies must be separated and distributed as follows: <br />The first original signed copy of each discharge monitoring report (DMR) shall be <br />submitted to the Division at the following address: <br />Colorado Department of Health <br />WQCD-PE-B2 <br />4300 Cherry Greek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be <br />submitted to the following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permittee records. The Discharge Monitoring <br />Report forms shall be filled out accurately and completely in accordance with <br />requirements of this permit and the instructions on the forms. They shall be signed <br />b}' an autl~ori2ed person as identified in Part I.D. <br />4. Acute 47ET Testin¢-Outfalls 002. 005 009 011. 013 and 015 <br />(a) Testin ¢, and Reoorting Requirements <br />Tests shall be done at the frequency listed in Part I.B.1. Test results shall <br />be reported along with the Discharge Monitoring Report (DMR) submitted for the <br />reporting period during which the sample was taken. (i.e., WET testing results <br />for the first calendar quarter ending March 31 shall be reported with the DMR <br />due April 28.) The results shall be submitted on the Acute Toxicity Test <br />report form, available from the Division. Copies of these reports are to be <br />submitted to both the Division and EPA along with the DMR. <br />Amendment 2 Issued Effective <br />See Part I.C. for Definitions. <br />
