<br />1. Discharge Monitorng Reoort: Battle Mountain Resources, InclBattle Mountain Gold Company must submit a
<br />Discharge Monitoring Report (OMR) on a monthly basis to the Division. This report should contain the
<br />required summary of the test results for parameters shown in Table VI-4 0l this rat/onale, and Part 1. B. of the
<br />permit. See the permit, Part I.E.1, (or details on such a submission.
<br />2. BMP Reoort _Battle Mountain Resources, Inc./BatUe Mountain Gold Company must submit a BMP Report on a
<br />quarterty basis to the Division. This report should contain the required summary of the results for monitoring
<br />shown to Tables VI-4A, VI-0B, and VI-4C of this rationale, and Part 1.8. of the permit.
<br />3. Soecial Reports: Special reports are required to be submitted to the Division in the event of a spill, bypass, or
<br />other noncompliance. Please refer to Part I.E.and Part 1. A.5 of the permit for reporting requirements
<br />C. Additional Terms antl Conditions
<br />1. Signatory and Certification Reouirements: Signatory and certification requirements (or reports and submittals are
<br />discussed in Part I.E.S. of the permit.
<br />2. Comoliance Schedules:
<br />a) Materials Containment Plan: The permittee will be required to submit a Materials Containment Plan. The
<br />plan shall address the prevention and containment of spills of materials used, processed or stored at the
<br />facility which, if spilled, would have a reasonable probability o/having a visible or otherwise dehimenta!
<br />impact on waters of the State. This plan fs to be submitted within 90 days of the effective date of the
<br />permi. See Part I.D. 1. of the permi.

<br />A) The permittee skull complete additional groundwater table elevation measurements at BF-3, BF-4, BF-5, BF-b, M-
<br />llR, M-l6, M-17, M-!8, M-l9, M-20, M-11, M-21, M-13, M-24, M-15, M-16, M-17, M-18, M-19, M-30. M-3l,
<br />M-31, and M-33 witkin 3 calendar days of the initial reposing of a back(! groundwater table elevation
<br />reading above 8581 ft amsl or a measured or calculated flow of groundwater jrom the West Pit. The
<br />groundwater table data wit! be used to calculate a new potentiometric surface map of the West Pit butt fll and
