Reoartin¢ of Data <br />Reporting of the data gathered in cc~liance with Part I.B.1 shall be an a quarterly basis. <br />Monitoring results shall be ~++~~~~ for each calerdar quarter axd reported on Division approved <br />discharge monitoring report (IY4t) forms (EPA form 3320-1). 'Lhe forms shall be mailed to the <br />agencies listed below so they are received m later than the 28th day of the month following the end <br />of the quarter (for en~ple, the DMR for the first calendar quarter mist be received by the Division <br />by April 28th). If m discharge occurs dicing the reporting period, "No Discharge" shall be <br />reported. <br />the DMR fonms consist of four pages - the top "original" copy, and three attached m-carbon-required <br />copies. After the R9t form has been filled out and sigred, the four copies mist be separated and <br />distributed as follows: <br />The first original signed copy of each discharge monitoring report (D~IIt) shall be submitted to the <br />Division at the following address: <br />Colorado Department of Health <br />GLZCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Derner, Colorado 80222-1530 <br />The first licate sigred copy of each discharge monitoring report (DSIIt) shall be submitted to the <br />following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch Bldf-C <br />999 18th Street, Suite 500 <br />Denver, ~ 80202-2466 <br />The third aid fourth copies are for the permittee records. The Disdharge Monitoring Report forms <br />shall be filled out accurately and cmpletely in accordance with requirements of this permit and the <br />instructions on the forms. They shall be sigred by an authorized person as identified in Part I.D. <br />4. Acute WET Testing-Outfalls 002. 005. 009. 011. 013. 015 and 016 <br />(a) Testing acd Reporting Requirements <br />Tests shall be dare at the frequency listed in Part I.B.1. Test results shall be reported <br />along with the Discharge Monitoring Report (IY•Ilt) submitted for the reporting period during <br />which the sample was taken. (i.e., WET testing results for the first calendar quarter ending <br />March 31 shall be reported with the DMR due April 28.) The results shall be submitted qn the <br />Acute Toxicity Test report form, available from the Division.
