The Division is required to make an approval <br />or denial decision no later than April 12, 1995 therefore, your <br />response to the following adequacy review concerns sf>ould be <br />submitted to the Division as soon as possible. If you feel more <br />time is needed to complete your reply, an extension to the decision <br />date can be granted by the Division. This will be clone upon <br />receipt of a written waiver of your right to a decision ;by April <br />12, 1995 and request for additional time. ~~' <br />1. Please change no. 12 on page 3 of the application';~Method of <br />Mining. The quarry designation applies to hard rock quarries <br />such as limestone operations.--~~Just cross out quarry and <br />briefly describe the operation in 12a, an example might be <br />"open pit gravel removed with dozer and loader and crushed." <br />2. Please show on the Pre-mining Plan map all immediate adjacent <br />surface owners of record;. <br />1 <br />i <br />3. Approximately how far ,up the exposed 50' highwall will soil <br />and topsoil. be piled and at what slope will the pile be <br />graded? -1 <br />In 1987, the U.S. congress amended the Clean Water Act to <br />expressly include storm water discharges from industrial sites <br />as requiring C1ean.Water Act controls. In 1990, EPA issued <br />regulations that require specific industrial activities to <br />obtain permits in-order to discharge storm water from their <br />facilities.
