This application cannot be approved until the remaining issues are <br />adequately addressed. Please submit a response to the following issues no later than June 25, <br />2001 so the decision due date of June 29, 2001 (extended from June 1 5`h at request of B and D <br />Trucking) can be met. If you cannot respond by June 25, 2001, Please submit a written request <br />for extension of the decision date. If your response, or an extension request, are not received by <br />June 25, 2001 this application may be denied. <br />ADEQUACY ISSUES <br />Application Form <br />1. Because the well is not to be installed for a few years, DMG suggests that, for bond <br />calculation purposes, B and D Trucking commit to an alternate reclamation plan that entails <br />revegetation of the entire area disturbed by the mining operation. This vegetation will serve to <br />stabilize the pit as a future pond site until the well and pump are installed and ready to <br />produce water and the well permit has been issued.
