PART I Page viii Permit No. CO-0000213 B. 114ONTfORING REQUIREMENTS 1, Frequency and Sample Tvpe (continued) (b) Oil and Grease Monitoring For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a grab sample shall be collected, analyzed, end reported on the appropriate DMR. In addition, corrective action shall be taken immediately to mitigate the discharge of oil and grease. A description of dte corrective action taken should be included with the DMR. 2. Reportin¢ of Data Reporting of the data gathered in compliance with Part I.B.1 sha[I be on a guarYerlY basis. Monitoring results shall be summarized for each calendar quarter and reported on Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed m the agencies listed below so they are received no later than the 28th day of the month following the end of the quarter (for example, the DMR for the first calendar quarter must be received by the Division by April 28th). If no discharge occurs during the reporting period, 'No Discharge' shall be reported. The DMR forms consist of (our pages -the lop 'original' copy, and three attached no-carbon-required copies. After the DMR form has been filled out and signed, the four copies must be separated and distributed as follows: The first original signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the following address: Colorado Department of Public Health and Environment WQCD-PE-B2 4300 Cherry Creek Drive South Denver, Colorado 80222-1530 The first duplicate signed copy of each discharge moaitoring report (DMR) shall be submitted to the following agency: U.S, Environmental Protection Agency Water Management Division NPDES Branch 8WM-C 999 18th Street, Suite 500 Denver, CO 80202-2466 The third and fourth copies are for the pemvttee records. The Discharge Monitoripg Report forms shall be filled out accurately and completely in accordance with requirements of this perari[ and the instructions on the forms. They shall be signed by an authorized person as identified in Part I.D. 3. Acute WET Testirt2-Outfalls 001 and 007 (a) Testier and Reportin¢ Requirements The first tests for outfalls 001 and 007 shall be performed during the first quarter of 1995. Tests shall be done at the frequency Jisted in Part 1.B.1. Test results shall be reported along with the Discharge Monitoring Report (DMR) submitted for the reporting period during which the sample was taken. (i.e., WET testing results for the first calendar quarter ending March 31 shall be reported with the DMR due April 28.) The results shall be submitted on the Acute Toxicity Test report (orm, aveileble from the Division.
