PART I <br />Page viii <br />Permit No. CO-0000213 <br />B. 114ONTfORING REQUIREMENTS <br />1, Frequency and Sample Tvpe (continued) <br />(b) Oil and Grease Monitoring <br />For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a grab sample <br />shall be collected, analyzed, end reported on the appropriate DMR. In addition, corrective action shall be taken <br />immediately to mitigate the discharge of oil and grease. A description of dte corrective action taken should be <br />included with the DMR. <br />2. Reportin¢ of Data <br />Reporting of the data gathered in compliance with Part I.B.1 sha[I be on a guarYerlY basis. Monitoring results shall be <br />summarized for each calendar quarter and reported on Division approved discharge monitoring report (DMR) forms <br />(EPA form 3320-1). The forms shall be mailed m the agencies listed below so they are received no later than the 28th <br />day of the month following the end of the quarter (for example, the DMR for the first calendar quarter must be received <br />by the Division by April 28th). If no discharge occurs during the reporting period, 'No Discharge' shall be reported. <br />The DMR forms consist of (our pages -the lop 'original' copy, and three attached no-carbon-required copies. After the <br />DMR form has been filled out and signed, the four copies must be separated and distributed as follows: <br />The first original signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the <br />following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge moaitoring report (DMR) shall be submitted to the following agency: <br />U.S, Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the pemvttee records. The Discharge Monitoripg Report forms shall be filled out <br />accurately and completely in accordance with requirements of this perari[ and the instructions on the forms. They shall <br />be signed by an authorized person as identified in Part I.D. <br />3. Acute WET Testirt2-Outfalls 001 and 007 <br />(a) Testier and Reportin¢ Requirements <br />The first tests for outfalls 001 and 007 shall be performed during the first quarter of 1995. Tests shall be done at <br />the frequency Jisted in Part 1.B.1. Test results shall be reported along with the Discharge Monitoring Report <br />(DMR) submitted for the reporting period during which the sample was taken. (i.e., WET testing results for the first <br />calendar quarter ending March 31 shall be reported with the DMR due April 28.) The results shall be submitted on <br />the Acute Toxicity Test report (orm, aveileble from the Division.
