RPR-08-1999 10 42 RGRPITO R950CIRTEB 970 245 9234 P.08
Draft: April 7, 1999 Page 6
attributed to ambient or external conditions, will be cause to initiate corrective action procedures.
The following sequence of corrective action procedures will be initiated.
1. Notify the BLMand the EPA, within the next BI,M working day, that the corrective
action sequence ofproceduresfnr change in hydrostaticpressure at the Dissolution
Suuface has been implemented.
2. Evaluate the integrity of the pressure transducer.
3. Evaluate other Dissolution Surface hydrostatic monitoring systems.
4. Evaluate the mass balance of the production wells in the azea.
5. Discontinue injection into the solution mining well and balance the density of the
fluids in the well.
6. Pressure test all tubing, strings, and annular areas between well strings with nitrogen
gas to hydrostatic pressure to ensure the mechanical integrity of the tubing.
7. Perform a temperature log to determine if there is any significant tempernture change
above the aahcolite-bearing wne. '
8. Evaluate the results from these procedures to determine the best solution for
wrrecting the problem. The use of additional logging techniques may be required
to accurately pinpoint any mechanical failures.
9. Notify the BLM and EPA of the results of the evaluation and the proposed cortective
actions to be taken.
6.9.3 ProcesslVlanitoring
Ifchanges in physical parameters at the production stream indicate aformation failures cavity
breach, or production well breach. The BLM and EPA will be notified within the next BLM working
day, and the following procedures will be initiated.
1. The well pressure, temperature and flow rate instrumentation will be checked for
accuracy.
2. The wel! valves will be checked for possible leakage.
3. injection into the solution mining well will be discontinued and the density of the
fluids in the well will be balanced.
Agapito Associates, Inc.
