The as-built haul road location, and the horizontal and vertical alignment is shown on Exhibit 13-16. <br />• The road width is adequate for the anticipated volume of traffic. <br />Density tests and proctors were taken to ensure proper compaction of all fill areas. These test results <br />can be found in Addendum D-1. All fill was placed in accordance with specifications found in Tab 13 <br />of the approved permit. The road structure appears to be stable, evidencing no indications of cracks, <br />deep rutting, settlement, or slope instability. Side cut slopes are 1.5H:IV or flatter and cut slope <br />stability is not a problem at this time. The fill slopes were constructed at a slope of 2H:IV or less <br />which should cause no stability problems. Some new cracks that were detected during the June 14, <br />2001 CDMG Inspection have formed in the vicinity of the outslope above Pond 012. These areas will <br />be reworked based on the recommendations contained in Attachment 13-12B. <br />To the best of my knowledge and belief, field adjustments made during the construction of the <br />structure are in conformance with the Rules and Regulations and the intent of the approved design <br />plans and specifications as approved by the Colorado Mined Land Reclamation Division. <br />JUN 2 "~ 2001 <br /> <br />u <br />Date <br />Revised 06/01 <br />2 <br />
