C HEROIF.F.SII'yIP <br />Semi-Annual Inspection: <br /> <br /> <br />\I.~S" 13. 1997 <br />The Stormwater Management Plan Administrator or his designee will perform an inspection at <br />this site on asemi-annual basis. The inspector will be looking for any evidence of the stockpiles <br />inability to contain all the rainfall runoff from the pit. He will also be observed the silt fence for <br />any sits of loss of integrity. <br />Should the inspector discover a problem with the stockpile containment or the integrity of the silt <br />fence, that problem will be corrected within 60 days. <br />If the inspector should observe an additional source of potential pollutants, the description of that <br />potential pollutant source and the pollution prevention measures identified in this plan shall be <br />revised as appropriate within two weeks of such inspection. Such revisions shat! provide for <br />implementation of any changes to this plan in a timely manner, but in no case more that 60 days <br />after the inspection. <br />Recordkeeping and Internal Reporting Procedures: <br />A report, summarizing the scope of the inspection, personnel making the inspection, the date of <br />the inspection, major observations relating to the implementation of the SWMP, and actions taken <br />in accordance with the previous paragraph, shall be produced and retained as part of the SWMP <br />for at least three years. The SWMP Administrator shall sign this report and certify its correctness <br />and completeness. <br />3.0 CERTIFICATION <br />This certification is provided by Gary Snook, Vice President of Operations for this BMI-France <br />owned refractory manufacturing plant, located at 309 South Eleventh Street in Canon City, <br />Colorado. <br />"I certify under penalty of law that this document and all attachments were prepazed under my <br />direction or supervision in accordance with a system designed to assure that qualified personnel <br />properly gathered and evaluated the information submitted. Based on my inquiry of the person or <br />persons who manage the system, or those persons directly responsible for gathering the <br />information, the information submitted is, to the best of my knowledge and belief, true, accurate, <br />and complete.
