At all times, including periods of start-up, shutdowq and malfimctioq the facility and control equipment shall, to the extent practicable, be maintained and operated in a manner wnsistent with good air pollution prevention and control practices for min~miring emissions. Determination of whether or not acceptable operating and maintenance procedures are being used will be based on information available to the Divisioq which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. 5. A revised APEN shall be submitted no later than 30 days before the five year term expires. 7. A revised Au Pollutant Emission Notice (ADEN) shall be filed when a significant change in actual emissions occurs (above the level reported on the last ADEN, submitted to the Division) as required by Regulation No.3, Part A.l7.C.2 as per the timetable in Regulation No.3; Part A.II.C.3. 8.
