Ru/e 2: Permits <br />• LCC or LCC's licensed and certified blasting contractor will record each blast, including <br />seismograph reports (as required). These records will be retained for at least 3 years and will be <br />available for inspection by the Division and by the public on request. The record shall contain <br />the following data: <br />(1) Name of the operator conducting the blast; <br />(2) Location, date, and time of blast; <br />(3) Name, signature, and license number of blaster-in-chazge and name and signature of one <br /> other person present at the firing of the blast; <br />(4) Identification, direction and distance, in feet, from the nearest blast hole to the nearest <br /> dwelling, school, church, or community or institutional building either: <br /> (a) Not located in the permit area; or <br /> (b) Not owned nor ]eased by the person who conducts the surface coal mining <br /> operations. <br />(5) Weather conditions, including temperature, wind direction, and approximate velocity; <br />(6} Type of material blasted; <br />(7) Sketches of the blast pattern including number of holes, burden, spacing, and delay <br /> pattern; <br />(8) Sketches will show decking, if holes are decked to achieve different delay times within a <br /> hole; <br />(9) Diameter and depth of holes; <br />(10) Types of explosives used; <br />(11) Total weight of explosives used per hole and maximum weight of explosives used per 8- <br /> millisecond period; <br />(12) Initiation system; <br />(13) Type and length of stemming; <br />(14) Mats or other protections used; <br />(15) Type of delay detonator and delay periods used; <br />(16) Number of persons in the blasting crew; and <br />(17) Seismographic and airblast records, where required, including: <br /> (a) Type of instrument, sensitivity, and certifications of annual calibration; <br /> (b) Exact location of instrument and the date, time and its distance from the blast; <br /> (c) Name of the person and firm taking the reading; <br /> (d) Name of the person and firm analyzing the seismographic record; <br /> (e) The vibration and/or airblast level recorded. <br />(18) Reasons and conditions for each unscheduled blast. <br />At least 30 days before initiation of blasting, LCC will provide written notification to Mr. Dickie <br />Hill and Colorado Interstate Gas, the owner and operator of the 10-inch natural gas pipeline <br />along the northern portion of the permit boundazy. The notification will explain the procedure to <br />request apre-blasting survey if the owner so requests. <br />LCC will give individually distinguishable warning and all-clear signals which will be audible <br />within 0.50 miles from the point of the blast. The pre-blast warning signals will be three reports, <br />each five seconds in length and will begin one to two minutes before each blast.
