You must certify that all discharges to state <br />waters have ceased. Upon receipt of the completed form, the <br />District Engineer for the above-referenced county will be notified <br />and will inspect your facility. Once the District Engineer has <br />approved inactivation, the permit will go to public notice for a <br />30-day comment period. Regulations require that monitoring reports <br />be submitted until the permit is formally inactivated. If <br />appropriate, such reports should simply state "no discharge". <br />Please be aware that Section 6.16.0(5) (B) of the permit <br />regulations states: <br />"Once the Division porceeds to terminate a permit at the <br />permittee's request, the prorated fee shall apply to the <br />period of time the permit has been in effect, including <br />but not exceeding ninety (90) days from the date the <br />permit termination request is received by the Division." <br />
