STATE HIGHWAY ACCESS PERMIT #399056 <br />Issued to Bowie Resources. Inc. <br />TERMS & CONDITIONS <br />I. This pemtit i$ issued in conlunction with Permits Ns 399055 and Ns 399057. Sub <br />considerations for all of these permit,. CDOT Form 1'_65 is required before beginning the construct on of the access or an}• activity <br />within the highway right-of-way. All submittals. documents, plans and other item that must be completed, shall <br />he submitted and approved before a Notice to Proceed will be issued. Contact the epartment (or local issuing <br />authority) for'the Notice to Proceed. <br />6. The followin~ items are required before a Notice to Proceed will be issued: <br />i(a) Consvuction Plans Stamped by a Colorado Registered Professio al Engineer. <br />Kb) Certificate of Insurance Liability as per Section 2.3(I I)(h) of th Access Code. <br />Qc) Traffic Control Plan <br />7. The ~Permitte¢ shall refer to all additional standard requirements on the back of thi permit and any enclosed <br />additional terms, conditions. exhibits and noted attachments. <br />8. This Permit i$ issued in accordance with the State Highway Access Code (2 CCR 01-1), and is based in part upon <br />the informatipn submitted by the Permittee. This permit is only for the use and pu pose stated in the Application <br />and Permit. Any changes in traffic volumes or type, drainage, or other operationa aspects may render [his permit <br />void, requirirl'g a new permit to be applied for based upon existing and anticipated uture conditions. <br />9. No highway lane closures or one-way traffic will be allowed. <br />16. No work will) be allowed at night, Saturdays, Sundays and legal holidays without <br />Department.
