This pennitl is issued in conjunction with Permits Ne 399056 and Ne :99057. Su mina) for desien shall include <br />considerations for all of these permits. The Permulee shall design and submit for appro~'al a left turn deceleration and a coloration lane 16 feet in width in <br />conformance with section 4 of the Smte Highway Access Code. 2CCR 601 <br />3. The Permittlee shall design and submit for approval a right turn deceleration and cceleration lane 12 feet in width <br />in conformance with section 4 of the State Highwa}• Access Code, 2CCR 601-I. <br />4. Design of this access with auxiliar}• lanes shall include, at a minimum, I t/x-inch verlay. <br />5. A Notice to Proceed, CDOT Form 1265 is required before beginning the constru [ion of the access or any activity <br />N•ithin the highway right-of-way. All submittals, documents, plans and other ire s that must be completed, shall <br />be submiued and approved before a Notice to Proceed will be issued. Contact th Department (or local issuing <br />authoriq') fqr the Notice to Proceed. <br />6. The followipg items are required before a Notice to Proceed will be issued: <br />(a) Construction Plans Stamped by a Colorado Registered Professi nal Engineer. <br />(b) Certificate of insurance Liability as per Section 2.3(1 1)(h) of t e Access Code. <br />(c) Traffic Control Plan <br />7. The Permitt<e shall refer to all additional standard requirements on the back oC th s permit and any enclosed <br />additional teams, conditions, exhibits and noted at[achments. <br />8. This Permit is issued in accordance with the State Hig}tway Access Code (2 CCR 601-1), and is based in part upon <br />the information submitted by the Permittee. This permit is only for the use and p tpose stated in the Application <br />and Permit.
