August 2002 -37- 003-2203 <br />I • - Verification of factory roll number and receipt of complete certification prior <br />to allowing deployment; <br />- Measurement of sheet thickness; <br />- Documentation of deployed panel lengths; <br />- Development of panel layout record drawing; and <br />- Observation of Phase IVa/Phase I tie-in. <br />Prior to the deployment of geomembrane, Golder and Poly-Flex verified that the subgrade was <br />free of soft spots, protrusions, angulaz particles, abrupt grade changes, and loose soil. Golder <br />and Poly-Flex verified that the SLF surface had been proof-rolled, and was free of desiccation <br />cracks. Where the surface of the SLF appeazed rough, Ames spread a thin layer of fines of <br />Portland Tailing from below Pad No. 3 over the azea. Poly-Flex provided written certification <br />of geomembrane subgrade acceptance. The subgrade acceptance certificates aze presented in <br />• Appendix G. <br />Golder assigned each geomembrane panel a consecutive number. Single-sided textured 80-mil <br />geomembrane panels deployed in the OSA were designated by P-1 through P-470. Golder <br />mazked the panel designation and roll number duectly on each panel after it was deployed. The <br />geomembrane record drawing, showing the geomembrane panel layout and destructive testing, <br />is presented in Record Drawing No. 5, Appendix A. <br />During geomembrane deployment, Golder measured sheet thickness with a micrometer. Golder <br />measured sheet thickness at five random locations along one edge of each geomembrane panel. <br />The average thickness specified for the 80-mil geomembrane was 80-mil. Project specifications <br />required that the average of the five thickness measurements be no less than the 80-mil, and that <br />no individual measurement be less than nominal thickness minus 8-miI (i.e, 72 mil).
