Certification Procedttres <br />If the general permit is applicable to the applicant's operation, then a certification will be developed and the applicant will be <br />certified undw this general pemtit <br />a. Request for Additional Infomtatlom The Division shall Lave up to thirty days after receipt of the above information to <br />request additional data and/or dew the authorization form particular discharge. Upon receipt of additional information, <br />tLe Division stall have an additional thirty days to issue or deny autLarization for the particular dischazge. (Notification of <br />denial stall be by letter, in cases when coverage user an altemal general pemtitoran individualpetarit is required,instead <br />of coverage under this general permit) <br />b. Automatic Coverage: If the applicant does not receive a request for additional informaion or a notification of denial from <br />the Division dated within thirty days of the receipt of the application by the Division, audorization to discharge in <br />accordance with the conditions of this permit shall be deemed granted. <br />c. Ltdividual Permit Required: L; after evah>ation of the application (or additional information, such as tlu SWIvIP), iris found <br />tLat this general permit is not applicable to the operation, then Ote application will be processed as one for an individual <br />permit The applicant will be notified of the Division's decision to deny certification under this general permit. General vs. Individual Permit Coverage: Any owner m operator authorized by this permit mayrequest to be excluded from <br />the coverage of this permit by applying for an individual CDPS permit Tht owner or operator stall submit an individual <br />application to the Division, with reasons supporting the request. <br />6. Permit Certifications. At+plicable Sections: <br />The individual certification prepared for each applicant will indicate whither PartI.B oftbe permit which must be followed by that <br />applicant Part I.C must be followed by all pemtitlea unless exempted as outlined in Section lII.C ofthe Rationale. (In flu absence <br />of such indication from the Division, the permittee wt71 be responstble for determining and then complying with fhc applicable <br />sections in Part I.) In addition, all nermittees will be required to complywith Part i, subparts A, D, E and F, and all of Part II of the <br />permit <br />Permit Expiration Date <br />Authorization to discharge under this general permit shall commence on October 1, 2002, and stall expire on September 30, 2007. <br />The Division must evaluate and reissue this general permit once every 5ve years, and must recertify tLe applicant's authority to <br />discharge under the general permit at sort time. Therefore, any permittee desiring continued coverage under the general permit <br />must reapply by MazcL 31, 2007. If the pettnittee's individual certification does not indicate that Part 1.B of the permirmust be <br />followed (i.e., the certification is for stormwatet discharges Daly), the petmittee ~tst reapply by June 30, 2007 if they desire <br />continued coverage under dte general permit The Division will initiate dte renewal process; however, it is ultimately the permittee's <br />responsibility to ensure that the renewal is submitted. The Division will determine iftlu applicanttnay continue to operate under the <br />terms of the general permit An individual permit will be regttued for any facility not reauthorized to dischazge under the reissued <br />general permit For facilities wishing to terminal authorization under flu newpermit, provisions ofPart II.B.S.d will be applicab]e. <br />8. Inactivation of Certification <br />The pemtittee may request inactivation of their certification under this permit, once the site Las bern fully released from then <br />reclamation bond requirements by the Colorado Division of Minerals and Geology (CDMG). See Part II.B.S.d of die permit. <br />
