Within 24 hours of the operator's detection of an emergency situation or condition as <br />specified in No. 1 (a) and (b) above, the permittee shall initiate all necessary and <br />appropriate measures to contain and detoxify cyanide solutions in the heap leach facility. <br />The permittee shall cease the application of cyanide~ontaining solutions to the heap <br />leach facility and shall take all measures that are necessary and appropriate to reduce or <br />minimize the solution head in the facility. If it can be clearly shown to the satisfaction of <br />the Office that the loss of containment is isolated to a specific portion of the heap leach <br />facility, and that that portion of the heap leach facility can be physically isolated from the <br />remaining heap leach facility as to cyanide solution application, the Office may give <br />approval for limited application of cyanide solutions to solution competent heap leach <br />facility areas. <br />e. As soon as practicable, after an emergency situation or condition is reported and <br />addressed, the permittee shall comply with the provision of Rule 8.2.3. <br />f. All fluids contaminated according to No. 1 (b) above in ground water monitoring wells <br />shall be collected, stored (not to exceed 60 days), detoxified to applicable surface water <br />standards, and released. All fluids contaminated according to No. 1 (a) above shall be <br />collected, stored (not to exceed 60 days), detoxified to applicable surface water standards, <br />and released. <br />g. Under no circumstances shall any of the fluids collected in No. 1 (f) be introduced into <br />the heap leach facility. The heap leach facility for the purpose of this condition shall <br />include the area delineated by the footprint of the valley fill heap leach. <br />h. Where data indicates conditions described in No. 1 (a) or (b) above, the permittee shall <br />monitor (sample and analyze) all sample points daily until the emergency situation or <br />condition defined by No. 1 (a) or (b) is shown to not exist. The emergency situation or <br />condition defined by No. 1 (b) will no longer exist if three consecutive ground water <br />samples taken five days apart from any sump that have WAD cyanide less than or equal <br />to .2 mg/I, and the pH measured on the same days and at the same sample points is less <br />than or equal to 8.5 pH; and if the five consecutive LDS samples taken 5 days apart from <br />any sump that have WAD cyanide less than or equal to .5 mg/I and the pH measured on <br />the same day at the same sample is less than or equal to 8.5. The operator shall remove <br />3 wells volume on each sample day prior to sampling for WAD cyanide and pH. <br />The emergency situation or condition defined by No. 1 (a) will not exist if three <br />consecutive surface water samples taken on three consecutive days have WAD cyanide, <br />pH, and flow rates equal to or less than what is defined by No. 1 (a) above; and if five <br />consecutive LDS samples taken 5 days apart have WAD cyanide less than or equal to .5 <br />mg/I, pH 8.5. <br />
