..... <br /> <br />NPDES General Permit for Stormwater Discharges <br />Associated with Industrial Activity <br />ANNUAL SITE COMPLIANCE EVALUATION <br />This form is to be completed at least once each calendaz year based on actual on-site <br />review of pollution control structures and procedures described in the Storm Water <br />Pollution Prevention Plan (SWPPP). <br />;The area to be evaluated consists of access roads and railroad spur lines which lie outside <br />_ • _. - ' of the boundaries of the mine's individual CPDES permits but within the overall mine - <br />permit boundary. <br />- s ..Upon completion of this evaluation, the certification at the end of the form should be <br />. .: completed by the Environmental Supervisor. This evaluation shall be filed in Appendix 3 <br />of the SWPPP and maintained on file for at least 3 years. <br />Location Mine Site: <br />Date: <br />Person Performing Evaluation: <br />Provide a brief answer to the following questions. <br />1. What is the overall condition of the structural controls for storm water runoff and <br />pollution control? Structural controls include culverts, sediment traps, riprap erosion <br />protection, check dams or drop structures, detention ponds or infiltration basins, <br />diversion ditches, and headgates or control valves. Identify any maintenance needs. <br />Identify culverts by numbers referenced on the site map. <br />2. Is there any evidence of increased potential for significant amounts of pollutants to be <br />discharged in association with storm water runoff? Any exposed material stockpiles <br />should be noted. <br />
