Reportine of Data <br />Reporting of the data gathered in compliance with Par[ I.B.1 shall be on a quarterly basis. <br />Monitoring results shall be summarized for each calendar quarter and reported on Division approved <br />discharge nbaftoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the <br />agencies listed below so they aze received no later than the 28th day of the month following the end <br />of the quarter (for exan~le, the DMR for the first calendar quarter must be received by the Division <br />by April 28th). If no dischazge occurs during the reporting period, "No Discharge" shall be <br />reported. <br />The DMR forms consist of four pages - the top "original" copy, and three attached no-carbon-required <br />copies. After the DMR form has been filled out and signed, the four copies must be separated and <br />distributed as follows: <br />The first original signed copy of each discharge monitoring report (DMR) shall be submitted to the <br />Division at the following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the <br />Following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch SWM-C <br />999 18th Street, Suite 500 <br />Denver, OJ 80202-2466 <br />The third and fourth copies are for the pennittee records. The Discharge Monitoring Report forms <br />shall be filled out accurately and completely in accordance with requirements of this permit and the <br />instructions on the forms. They shall be signed by an authorized person as identified in Part I.D. <br />4. Acute WE't Testing-Outfalls 001 002 005. 009, 011. 013, 015, 016 and 017 <br />(a) Testing and Reporting Requirements <br />Tests shall be done at the frequency listed in Part I.B.1. Test results shall be reported <br />along with the Discharge Monitoring Report (DMR) submitted for the reporting period during <br />which the sample was taken. (i.e., WE1' testing results for the first calendar quarter ending <br />March 31 shall be reported with the CMR due April 28.) The results shall be submitted on tl~e <br />Acute Toxicity Test report form, available from the Division.
