local regulatory agencies shall be the responsibility of the <br />Nine hianager. <br />Any oil spill will be immediately reported to the Water Quality <br />Control Division, Colorado Department of Health, by the responsible <br />person, firm or corporation. Upon discovery of the release, an <br />initial telephone notification shall be made to the Division, which <br />will include the following information. <br />a. Responsible.company/person, including mailing address <br />and telephone number; <br />b. Name of reporting official; <br />c. Date and time of release; <br />d. Legal description o1 release location; <br />e. Type of substance; <br />f. Amount of released substance; <br />g. Watervray alfected, including amount reaching water; <br />h. Cause of release; <br />i. Action taken .to contain, control and remove release; <br />j. Other pertinent. information specific to the release. <br />A follow-up report will be written within seven C7> days follow- <br />ing the release. The Kine Manager shall prepare a complete <br />written report describing aft aspects of the release and steps <br />taken to prevent a recurrence. If circumstances make a complete <br />written report impossible within the seven day period, a partial <br />report shall be submitted. If a partial report has been <br />submitted, a final report will be sent to the Water Quality <br />Division when all details of the incident are available. The <br />final report will include, but not necessarily be limited to, the <br />following information: <br />a. Name of responsible company/person; <br />b. Date, time and legal description of release location; <br />c. Amount and type of oil or hazardous substance released; <br />-2- <br /> <br />
