PART I Page 15 of 20 <br />Permit No. COR-040000 <br />F. TERMINATION OF PERMIT COVERAGE (wnt.) <br />2. Request for Termination <br />When a site has been reclaimed in accordance with the CDMG requirements and the site haz been released from <br />State reclamation requirements, or the above Termination Criteria have bcen met, the permittce may request <br />cenification termination. The Division will consider termination of permit coverage when: <br />a. the permittce has submitted a completed Inaaivazion Form az provided by the Division which summar;~es the <br />work accomplished; <br />b. In the raze of mine-waste remediation projects, the permittce haz submitted photographic documentation of the <br />post remediation wnditians at the mine site; <br />c. All stormwater discharges to state wa[ers azsociated with mining activities have ceazed and the permittce haz <br />demonstrated to the Division the termination criteria have bcen met or the CDMG performance bond has been <br />releazed; <br />d. The Division fords that the permittce has shown reazonable grounds consistent wiUr the Federal and State <br />statutes and regulations for such termination; and <br />e. Fee requirements of Section 6.16.0 of State Discharge Permit System Regulazions have been met. <br />Continued coverage may be required after reclamazion or remediazion haz been completed if the Division has shown or <br />has reazon to suspect that the stormwater discharge may contribute to a violazion of a water quality standazd. <br />G. ADDITIONAL DEFINIITONS <br />1. Best management praMices (BMPs): schedules of activities, prohibitions of practices, maintenance procedures, <br />and other management practices to prevent or reduce the pollution of wazers of the Stare. BMPs also include <br />treatment, operating procedures, and pracices to control site mnoff, spillage or leaks, waste disposal, or drainage <br />from material storage. BMPs include stmctural and nonstnicnual controls. <br />2. Operator: the individual who has day-today supervision and control of activities occurring at the mine site. <br />3. Remediation, az used in this permit, means aaions taken to reduce or eliminate the overall adverse environmen[al <br />impact azsociazed with a pollttant source. The term includes actions such az re-routing of surface flows, re-location <br />or a pollutant source, the addition of pollutant neutralizing or immobilizing substances, capping and re-vegetation. <br />4. Significant Materials include but are not limited to: raw materials; fuels; materials such az metallic products; <br />hazardous substances designated under section 101(14) of CERCLA; fertilizers; pesticides; and waste products such <br />az ashes, slag and sludge that have the po[ential to be released with stormwater discharge. <br />5. Vegetative Cover is the aerial projection of all grasses, forbs, mosses (living or dead) onto the ground. <br />PART I[ <br />A. MANAGEMENT REQUIREMENTS <br />1. Chanee in Discharee <br />The permittee shall inform the Division (Permits and Enforcement Section) is writing of any intent to construct, <br />install, or alter any process, facility, or activity thaz is likely to result in a new or altered discharge, and shall <br />furnish the Division such plans and specifications which the Division deems reasonably necessary to evaluate the <br />effect on the dischazge and receiving stream.
