PART I <br />Page No. 12 <br />Permit No. COG-850000 <br />GENERAL MONTtORING, SAMPLING AND REPORTING REQUIItEMENTS <br />1. Routine Reportine of Data <br />Reporting of the data gathered in compliance with Part I.B.3. shall be on a auarterty basis. Reporting of all data <br />gathered shall comply with the requ'vemenu of Pan I.D. (General Requiremmu). Monitoring results shall be <br />s,irt,man~rn for each calendar gttaner and reported on Division approved discharge monitoring Iepon (DMR) forms <br />(EPA form 3320-1). The forms shall be mailed to the agencies listed below so thry aze received no later than the 28dt <br />day of the month following the end of the quarter (for example, the DMR for the first calmdaz quarter must be received <br />by the Division by April 28th). If no d'uchazge occurs during the reporting period, 'No Discharge' shall be reported. <br />The DMR forms consist of four pages -the [op 'original' copy, and three attached nocarbon-required copies. Afrer the <br />DMR form has bem filled out and signed, the four copies smut be separated and distributed as follows: <br />The first orieinal signed copy of each dschazge monitoring report (DMR) shall be submitted to the Division az the <br />following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />The US Environmental Pro[ection Agency Region VIII no longer rcquues submittal of duplicate wpia of the DMR to <br />thew office, therefore, the second, third and fourth copies arc for the permittee records. 'Ihe Discharge Monitoring <br />Report forms shall be filled out accurately and completely in accordance with requirements of this permit and the <br />instnraions oa the forms. They shall be signed by an authorized person as identified in Pan I.D.7. <br />Calculations for all limitations which require the averaging of n,e~v+ren,Pnu shall utilire an arithmetic mean unless <br />otherwise specified by the Division in the permit. <br />2. Reportina - Stormwater Annual Report <br />The permittee will be required to submit an Annual Report, wvetimg January 1 through December 31 of each yeaz, on <br />the overall compliance with the SWMP. The Annual Report will contain, az a ++++++~*+++~~+: <br />a) Name of permittee, address, phone number, and permit catifiration number. <br />b) A report on the facility's overall compliance with the SWMP. <br />c) A summazy of each comprehensive stormwater facility inspection made, including date, 5adiags, and action taken. <br />d) Results and intecprctation of any stormwater monitoring performed. <br />e) The report shall be signed and certified for accuracy by the permittee, imduding the cemficarion language contained <br />in Part I.D.7. of the permit. <br />The Annual Report will be due to the Division on or before February 15 of each yeaz, after the fast full yeaz of <br />coverage under the permit. The exact due date for the permittee's fast Annual Report will be listed im their permit <br />certification. The Division reserves the right to requtre additional information in the report, on a case-bycase basis, as <br />needed. <br />All teporu required for submittal shall be signed and certified for acatraty by the permittee (see Part [.D.7.). <br />
