Phase IVc <br />Ames performed confirmatory drilling activities associated with the Phase Nc construction azea on <br />known underground working UG-504. <br />Ames continued blast drilling of underground workings within Phase Nc at UG-288, UG-289, and UG- <br />270. Ames blasted underground workings delineated by confirmation drilling at UG-288/289 on May <br />21St at 16:30 and at UG-270 on May 23~ at 16:30. <br />(See Attachment A for approximate construction locations for all Phase N azeas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were is <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. <br />Golder observed recontouring of the Phase III ore slope. Ore was regraded to approximately 4H:1 V <br />and at no time was allowed off of the existing liner system. <br />Golder observed construction of the Primary Underdrain and Primary Underdrain Sump in Phase <br />IVa for trench dimensions (5' by 3'), slope (minimum 1%), geotextile (12 oz.), UF, geotextile <br />overlap (2'), SSF, wire mesh, and concrete. <br />• Golder observed construction of the Phase IVa Leak Detection trench for trench dimensions, <br />geomembrane welds, geotextile overlap, and LDF. Due to failing gradation on LDFa-Ol, Ames was <br />required to remove all of the installed LDT. Ames re-installed the LDT after a passing LDF results <br />were obtained (LDFa-02). <br />Golder provided observation of DCF removal from the edge of the Phase I geomembrane for defects <br />outside of the Phase IV tie-in. Defects outside of the Phase IV tie-in were mazked for repair. <br />Golder observed and tested placement of SLF in Phase IVa completing fifteen in-situ density tests, <br />nineteen depth verification tests, and verified surface preparation. Soil Liner Fill was placed in <br />accordance to the specification. <br />Golder observed deployment of geomembrane documenting ambient air temperature, panel <br />dimensions, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in Phase Na met specifications. <br />Golder observed and documented confirmation drilling activities associated with the Phase IV <br />construction. Full time monitoring was given to confirmation drilling of underground workings <br />including logging holes, mazking drill locations, surveying holes, and documenting underground <br />workings encountered. <br />• eiw~rv,s<M„~9-m~~~m~ Golder Associateslnc. <br />
