and at no time was allowed off of the existing liner system. <br />Golder observed construction of the Primary Underdrain and Secondary Underdrains in Phase IVa <br />. and Phase Nb for trench dimensions (5' by 3' and 4' by 2'), slope (minimum 1%), geotextile (12 <br />oz.), UF, geotextile overlap (2'), and SSF. <br />Golder provided observation of DCF removal from the edge of the Phase I and Phase III <br />geomembrane for defects outside of the Phase IV tie-in. Defects outside of the Phase Ntie-in were <br />mazked for repair. <br />Golder observed and tested placement of SLF in Phase IVa completing twenty-six in-situ density <br />tests, four depth verification tests, and verifying surface preparation. Soil Liner Fill was placed in <br />accordance to the specification. <br />Golder observed deployment of geomembrane documenting ambient air temperature, panel <br />dimensions, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in Phase IV a met specifications. <br />Golder observed and documented confirmation drilling activities associated with the Phase IV <br />construction. Full time monitoring was given to confirmation drilling of underground workings <br />including logging holes, marking drill locations, surveying holes, and documenting underground <br />workings encountered. <br />Golder observed placement of CSB, CP, and CR. These materials were placed in accordance with <br />the drawings and specifications into excavated or open underground workings. Materials supplied <br />• met specifications. Ground Engineering conducted field testing and sampling of CP concrete as per <br />specifications, and will supply 28-day strength results. <br />Golder observed and documented excavation and remediation activities associated with the Phase N <br />underground working construction. Full time monitoring was given to excavation of underground <br />workings. <br />Golder observed placement of geogrid in Phase IVb. Geogrid was placed in accordance with the <br />drawings and specifications over the designated area. <br />Laboratory Test Results <br />Golder obtained sample SLFa-O1 from Soil Liner Fill placed in the Phase IVa azea and began <br />laboratory testing. SLFa-O1 met the requirements of the specifications and results aze presented in <br />Attachment B. <br />Meetings <br />CC&V held Weekly Contractors meetings on May 7s' and May 14a'. Items discussed include Safety, <br />Environmental, Schedule, and CQA.
