Golder observed and tested placement of SLF in Phase Na, completing thirty in-situ density tests, <br />fifteen depth verification tests, and verified surface prepazation. Prior to geomembrane deployment, <br />the SLF met the requirements of the specifications. <br />Golder observed anchor trench excavation for proper location and dimension (2' wide and 3' deep). <br />Anchor trench excavation met or exceeded the requirements of the specifications. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in the Phase IVa area met the project specifications. <br />Golder observed construction of the Primary and Secondary Underdrains in Phase Nb for trench <br />dimensions (5' by 3' and 4' by 2'), slope (minimum 1%), materials, and geotextile overlap. <br />Golder observed and documented excavation and remediation activities associated with the Phase IV <br />underground working construction. <br />Golder observed and documented confirmation drilling activities associated with the Phase IV <br />construction. Full-time monitoring was given to confirmation drilling of underground workings, <br />including logging holes, mazking drill locations, surveying holes, and documenting underground <br />workings encountered. <br />Golder visually observed the placement of DCF in the Phase IVa azea. Golder had one CQA monitor <br />per dozer to observe DCF placement. <br />Laboratory Tests <br />Golder obtained four samples of SLF (SLFa-08 through SLFa-11) for laboratory testing. The <br />samples tested met the requirements of the specifications and results are presented in Attachment B. <br />Meetin¢s <br />CC&V held a Weekly Contractors meetings on July 16ei and July 23`d. Items discussed included <br />Safety, Environmental, Schedule, and CQA.
