Show aU operations conmbuting <br />wastewater, utcludmg process and production areas. The water balance should show average flows and all significant kisses of water to products, t <br />atmosphere, dust control and discharge. Use actual measurement whenever posstbe, otherwise use your best estunate. If a water balance is not <br />appropriate, Please indicate. <br />Item 14 - Mark which of the other permitslprograms the facility has obtained or isvt the process of obtaining or is subject to regulation under. <br />See items 37 and 38 regarding Stornwater requirements. <br />Item 15 - Selfexplanatory. <br />Item 16 - List here those chemicals or materials added to the water prior [o, during, and after the water is used and prior to discharge. Also list chemicals or <br />materials used in [he treatment of the wastewaters. <br />Item 17 - Tf apphcabk, list those toxic products thffi are used in the manufacturing process or are produced by the manufacturing process. Do not include the <br />substances listed in item 16. <br />Item 18 - Self explanatory. <br />Item 19 - List any improvements [o the wastewater system which ate tequued or which are being undertaken to ensure future compliance with environmenC <br />regulations.
