Savage, on December 30, 1996, I am sending you the form <br />which must be signed by you as you are listed as the legal contact. Before the Division can consider <br />inactivating the above-referenced permit, a release of bond from Division of Minerals and Geology is <br />required. At the time the bond is released, you can notify the Division for a termination form [o be completed <br />and rammed to this office. At that time you must certify that all discharges to state waters have ceased. <br />Upon receipt of the completed termination form, the District Engineer (D.E.) for the above-referenced county <br />will be notified and will inspect your facility. Once the D.E. has approved termination, the permit will go to <br />public notice fora 30-day comment period. Regulations require [hat monitoring reports be submitted until <br />the permit is formally inactivated. If appropriate, such reports should simply state "no discharge". <br />Please be aware that Section 6.16.0(5) (B) of the permit regulations state: <br />"Once the Division proceeds to terminate a permit at the permittee's request, the prorated fee shall <br />apply to the period of time [he permit has been in effect, including but not exceeding ninety (90) <br />days from the date the permit termination request is received by the Division." <br />The Division's Fiscal Officer has been notified of your termination request. Please contact Connie Moreno at <br />(303) 692-3507 with any questions concerning fees.
