j. Indicate the de~h and thickness of the coal, s~, gravel, quarry <br />aggregate, or limestone used for construction purposes, to be <br />mined and the thickness and type of the overburden to be removed. <br />80-90' <br />k. Indicate the expected physical appearance of the area to be mined <br />and the area of land affected, correlated to the timetable required <br />herein. See attached. <br />14. Permit fee: <br />A fee of fifty dollars ($50.00) plus fifteen dollars ($15.00) for each <br />acre or fraction thereof of the area of land to be affected by the <br />operation must accompany this application. <br />$ 230.00 <br />(Amount of money submitted) <br />15. Bond: <br />A bond must be submitted together with Board Form 2. (You will be <br />advised in writing of the amount of the bond after this application <br />has been reviewed.) <br />a. The bond must be payable to the State of Colorado. <br />b. The bond must be conditioned upon the operator faithfully performing <br />all requirements of Chapter 92, Article 13, CRS 1963 as amended. <br />c. The bond shall be signed by the operator as principal and by a good <br />and sufficient corporate surety authorized to do business in the <br />State of Colorado. <br />d. Said bond must be submitted with the application unless the Land <br />Reclamation Board authorizes the operator to use one of the alterna- <br />tive plans set out in Section 92-13-8, CRS 1963 as amended. <br />16. Aland stabilization and reclamation agreement may also be required <br />pursuant to the provisions of Section 92-32-5, CRS 1963 (1969 Perm. Cum. <br />Supp.) <br />17.
