List and identify by number and land area any state permits which are <br />presently held or have been held in the past by applicant or any person <br />or legal entity affiliated or associated with applicant: <br />'?one <br />10. Give a detailed description of the method of operation to be employed in the <br />open mining: <br />Omen pit excavation using rubber tired front end loaders to remove <br />materiel from the excavation face end to transport to screening, and <br />•:ashina plant. Processed materials stockpiled using radial stacker <br />or front end loaders, <br />11. Attach two copies of an accurate map of the area affected by the opening <br />mining which map shall include, but not be limited to, the following: <br />a. The map shall be made and certified to by a registered land surveyor, <br />professional engineer registered in the State of Colorado, or a <br />qualified person. ~~tails ine'_uded.
