PART I <br />Page No. 12 <br />Permit No. COG-850000 <br />D. GENERAL MOMTORING, SAMPLING AND REPORTING REQUIREMENTS <br />1. Routine Reoortine of Data <br />Reporting of the data gathered in compliance with Part I.$.3. shall be-on a quarterly basis. Reporting of all data <br />gathered shall comply with the requirements of Part I.D. (General Requirements). Monitoring results shall be <br />s++++++++~~>r~ for each calrndar quarter and [[:ported on Division approved discharge monitoring report (DMR) forms <br />(EPA form 3320-1). The forms shall be mailed to the agrnci« listed below so thry aze received no later than the 28th <br />day of the month following the end of the quarter (for example, the DMR for the fast calendaz quarter must be received <br />by the Division by April 28th). If no dischazge occurs during the reporting period, "No Dischazge" shall be reported. <br />The DMR forms consist of four pag« -the top 'original' copy, and three attached nocarbon-required copi«. After the <br />DMR farm has been filled out and signed, the four copi« must be separated and distributed as follows: <br />The first on ' signed copy of each discharge monitoring report (DMR) shall be submitted to die Division at the <br />following address: <br />Colorado I)epaztmrnt of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />The US Environmental Protection Agency Region VIII no longer requir« submittal of duplicate copi« of the DMR to <br />their offix, therefore, the second, third and fourth copi« ate for the pertnittee records. The Dischazge Monitoring <br />Report forms shall be filled out accurately and completely in aaardanoe with requirements of this permit and the <br />instrutxions on the fortes. They shall be signed by an auffiorired person as identified in Part I.D.7. <br />Calculations for all limitations which require the averaging of mega+~*^+^+ts shall utilize an arithmeric mean unless <br />otherwise specified by the Division in the permit. <br />2. Reportine - Stotvnwater Annual Retort <br />The permittee will be required to submit an Annual Report, covering ]anuary 1 through December 31 of each yeaz, on <br />the overall compliance with the SWMP. The Annual Report will contain, at a minimum: <br />a) Name of permittee, address, phone number, and permit xrtificazion number. <br />b) A report on ilte facility's overall compliance with the SWMP. <br />c) A summary of each comprehensive stormwater facility inspection made, including date, findings, and action taken. <br />d) Results and interpretation of any stormwater monitoring performed. <br />e) The report shall be sighed and certified for accuracy by the petmittee, including the certificazion language contained <br />in Part I.D.7. of the permit. <br />The Annual Report will be due to the Division on of before February 15 of each yeaz, after the first full yeaz of <br />coverage under the permit. The exact due date for the permittee's fast Annual Report will be listed in their permit <br />certification. The Division reserv« the right to require additional information in the report, on a case-bycase basis, as <br />needed. <br />All reports required for submittal shall be signed and certified for accuracy by the permittee (see Part I.D.7.). <br />
