PART I <br />Page 14 <br />Permit No. COG-500000 <br />D. MONITORING AND REPORTING <br />10. S~natarv Reauiratrents <br />a. All reports required for submittal shag be signed and certified for accuracy by the permittee in accordmce with <br />the following criteria: <br />1) In the case of corporations, by a principal exautive officer of at leaf the level of vice-president or his or <br />her duly authorized representative, if such representative is responsible for the overall operation of the <br />facility Cram which the discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the propriemr; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, molting <br />elected official, or other duly authorized employee. <br />b. Changes to authorization. If as authorization under paragraph a) of this sation is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorization <br />satisfying the requirements of paragraph a) of this section must be submitted to the Division, prior to or together <br />with any reports, information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document under paragraph a) of this sation (except for the SWMP <br />certification) shall make the following certification: <br />'I certify under penalty of law that this document and all attachments were prepared under my direction <br />or supervision in accordance with a system designed to assure that qualified persomel properly gather <br />and evaluate the information submitted. Based on my inquiry of the person or persons who rttanage the <br />system, or those persons directly responsible for gathering, the information submitted is, to the best of <br />my Imowledge and belief, true, accurate, end complete. I am aware that there are significant penalties <br />for submitting false information, including the possibility of fine and imprisonment for knowing <br />violations.' <br />E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES <br />a/ The thirty (30) day average shall be determined by the arithmetic mean of all samples collated during a thirty (30) <br />coosecutive-0ay period. Samples shall not be used for more than one (1) reporting period. <br />b/ The seven (7) day average shag be determined by the arithmetic mean of all samples taken in a seven (7) day period. <br />Samples may not be used for more than one (I) reporting period. <br />c/ This limitation shall be determined by a single sample or set of samples ns required by Part I.B.3, Sample Type. <br />d/ Monitoring is required only during periods of discharge. If no discharge occurs, this shall be reported at the spaified <br />frequency. (See Part I.D.3.) <br />e! TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as <br />elatrical conductivity where a satisfactory correlation with TDS has ban established based upon a minimum of five <br />(5) samples. See the individual Certification Rationale for a determination as to whether monitoring is required. <br />f/ Phosphorus monitoring is required only for facilities which discharge into the following drainage basins: Cherry Creek <br />basin, Chatfield Reservoir downstream of the USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e., <br />Ten Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the <br />individual Certification Rationale for a determination as to whether monitoring is required. <br />
