(See Part I.D.10 of the permit.) a. Quarterly Report - Process Water For facilities allowed to discharge process water, reporting of the data gathered in compliance with Part I.B.3.a shall be oD a quarterly basis. Monitoring results obtained during the previous 3 months shall be summarized and reported on Division approved Discharge Monitoring Report forms (DMRs), postmarked ao later than the 28th day of the month following the completed calendar quarter. For example, for the 1st quarter (January, February, and March) the quarterly report must be received at this office postmarked ao later than April 28. If no discharge occurs during the reporting period. 'No Discharge' shall be reported. b. Amual Report -Process Water For facilities not allowed to discharge process water (see Part I.B.3.b of the permit), a report on whether or not a discharge of process water has fatten place at the facility is requited, iD letter form, on aD annual basis. The report shall cover October 1 through September 30 of each year, and will be due to the Division by the following October 28, begiming with the first year or partial year of permit wversge. The Division reserves the right the require additional iDformation in the report, on a rase-by~ase buffs, u needed. 4. Reportin¢ of Data - Stormtsater Dischareers e. Annual Report - SWMP The permittee will be required to submit an amual report, covering ]antrary 1 through December 31 of each year, on the overall compliance with the SWMP. The annual report will contain, at a minimum: 1) Name of permittee, address, phone Dumber, and permit certification number. 2) A report on the facility's overall compliance with the SWMP. 3) A summary of each comprehensive stortnwater facility inspection made, including date, findings, and action taken. 4) Results and interpremtioD of any storrmvater monitoring performed. The amual report will be due to the Division on or before February 15 of each year, after the first full year of coverage under the permit. The exact due date for the permittee's first amual report will be listed is their permit certification. In ao case will an amual report be due before February 15, 1994, unless specifically
